How to Apply

To determine which of our programs accommodates you the best, please fill out the online application, and send the following documentation to our office:

• Two months of bank statements
• Two months of paycheck stubs
• Last two years of tax statements and W2 forms
• If self-employed bring in income tax returns for the previous two years and a year to date profit and loss statement
• Letter of non-traditional credit if applicable (Ex: Utilities, auto insurance, etc.)

Note: The completion of this form does NOT constitute a formal application for credit. This form is used solely for determining program eligibility. If eligible, you will be asked to complete a formal application.

Please know that a fee of $18.00 to cover the cost of your credit report is required for us to process your intake form. Please make your check or money order payable to NHS of San Antonio and write your name on the check. You may also bring the payment by our office at:
